Chronic Disease Management Live Panel Discussion

Did you know that in Ontario, 44% of adults over 20 years old have at least one of ten common chronic diseases? Living and managing such a disease can be extremely difficult, and feeling in control of it can be a long process.

In Canada, chronic diseases are the cause of 65% of all deaths each year and are the leading causes of death globally. And yet, many of these conditions are preventable. Cancers, cardiovascular diseases, chronic lower respiratory diseases, and diabetes represented an estimated annual direct health care cost of $10.5 billion.

Having members with chronic diseases also has hidden costs that take the form of absenteeism and disability.

Before the pandemic, getting help and answers was already complicated. But now, with the health care system under extreme pressure and appointments, tests and surgery being delayed, your members and employees need somewhere to turn to more than ever. Help them by giving them what they need – immediate help.

Our panel discussion will be held by our team of health professionals that work, day in, and day out, with patients in different settings. They will help you understand what chronic diseases are, how to prevent them, and what care coordination is as well as chronic disease management.

Panelists

Dr. Michael Szabo

Michael is the Chief Medical Officer at Novus Health. He is an emergency physician at University Health Network in Toronto and a lecturer in the Department of Family and Community Medicine at the University of Toronto. Dr. Szabo has over 20 years’ experience providing front-line medical care. He also has an MBA from the Richard Ivey School of Business at Western University.

 

 

 

Ines Puentes

Ines Puentes is the Director of Clinical and Wellness Programs at Novus Health. Ines holds a bachelor’s degree in Immunology and a master’s degree in Nursing Sciences, both from McGill University. In her 20 years’ experience as a nurse clinician and team leader at community health centres in the greater Montreal area, she has acquired vast experience in the implementation of health promotion and prevention programs.

 

 

 

Erin Kemp

Erin Kemp is a Nurse and Patient Care Coordinator at HealthCare 365. She has worked in multiple care settings throughout the private and public health care sector making her an expert in health systems navigation. Coordinating care for hundreds of patients with chronic conditions, she is proactive in the field of health promotion & management. Erin holds a Nursing Diploma from Durham College and is studying to specialize in gerontology and mental health.

Pauline is the Director of Client Relations and Development for Novus Health. She draws upon extensive experience working with government and public organizations as well as innovative tech companies, both in North America and in Europe, to provide an exceptional client experience to our customers and support our growth.
