Health Care Services International Inc., carrying on business as Novus Health, (“HCSI”) and its affiliates are committed to protecting your privacy. This Privacy Policy applies to the collection, use and disclosure by HCSI and its affiliates of information about identifiable individuals (“Personal Information”) collected through or in relation to this website (the “Site”).

1. Collection of your Personal Information
HCSI may collect Personal Information, such as your name, email address, contact name, company name and address, so you may be provided access to the Site (the “Registration Information”). Other Personal Information will also be collected by HCSI when you voluntarily choose to supply it through the Site.

HCSI may collect information about your visit to the Site, including the pages you view and the links you click. HCSI also collects certain standard information that your browser sends to every web site you visit, such as your IP address, browser type and language preference, access times and referring web site addresses. This information will not be shared by HCSI or its affiliates.

Personal health information will be collected on the Site only if the “personal health record” feature (as shown on the Site) is activated.

2. Use of your Personal Information
HCSI uses your Personal Information to create a user account so you can access the Site. Your Personal Information is also used to assist HSCI in delivering customized services and products to better meet your needs.
HCSI may use your Personal Information to verify that you are authorized to use the Site. HCSI may also use your Personal Information to generate statistics and reports for internal use, and to evaluate the performance and usage frequency of the Site.

3. Disclosure/Transfer of Your Personal Information
HCSI will not sell, rent or lease your Personal Information to any third parties.
HCSI may access, use and/or disclose your Personal Information if required or permitted to do so by law.
HCSI may also access, use and/or disclose your Personal Information under the following circumstances:
(a) to comply with a legal requirement including but not limited to one imposed by a warrant, subpoena, court order or like instrument served on HCSI or its affiliates;

(c) in urgent circumstances to protect the life, health or security of any person.
Other than as set forth above, HCSI will not use or disclose your Personal Information without your consent. Any use or disclosure of your Personal Information within HCSI will remain subject to the limitations of this Privacy Policy. HCSI will give you notice and obtain your consent before it uses or discloses your Personal Information for any purpose that is not described in this Privacy Policy.

HCSI may also disclose your Personal Information to its successor or any assignee, as necessary, without notice to you.

4. Security of your Personal Information
HCSI is committed to protecting the security of your Personal Information. HCSI uses a variety of security and information technologies and procedures to help protect your Personal Information from unauthorized access, use, or disclosure.

5. Use of Cookies
HCSI uses “cookies” to facilitate your online experience. A cookie is a text file that is placed on your hard disk by a web page server. Cookies cannot be used to run programs or deliver viruses to your computer. Cookies are uniquely assigned to you, and can only be read by a web server in the domain that issued the cookie to you.

One of the primary purposes of cookies is to ensure that you are authorized to use the Site. Following each of your sessions on the Site, any cookies from the Site are deleted.

6. Aggregate Data
HCSI may provide Aggregate Data to third parties. “Aggregate Data” means similar pieces of non-personal information (i.e., information that cannot be used to identify a particular individual) that are gathered by the Site from two or more users of the Site and are combined.

7. Changes to this Policy
HSCI encourages you to periodically review this Privacy Policy. This Privacy Policy may occasionally be updated or changed. When changes are made to this Privacy Policy, you will see the word “updated” next to the Privacy Policy link on the introductory page of the Site for a period of at least thirty (30) days from the date of the update. The date that this Privacy Policy was last updated will appear at the top of the Privacy Policy.
